A necrotizing gas-forming infection affecting the renal parenchyma, urinary tract, and perirenal tissue is referred to as emphysematous pyelonephritis (EPN). Patients with diabetes mellitus (DM) often have it. Early, intensive bacterial treatment can avoid nephrectomy in EPN. In this report, we describe an EPN patient with unique imaging and a great prognosis following early diagnosis with solely supportive and antimicrobial treatment. Emphysematous pyelonephritis (EP) is a fairly rare complication of acute pyelonephritis (AP), in which gas-forming bacteria (usually intestinal bacilli) ferment glucose to carbon dioxide and water, thereby causing an acute necrotizing process in the renal parenchyma [1,2].
