ENDOSCOPIC CLASSIFICATION AND HISTOLOGICAL CHARACTERISTICS OF PRECANCEROUS PATIENTS WITH GASTRIC CANCER

Abstract
Precancerous gastric conditions (atrophy, intestinal metaplasia, dysplasia) show subtle endoscopic signs like vague borders, irregular patterns, color changes, and specific microvascular changes visible with magnifying NBI, while histologically they involve glandular loss (atrophy), goblet cells (metaplasia), and cellular atypia (dysplasia), progressing towards gastric cancer with more defined irregular features and architectural changes. The key is differentiating these from actual early cancer (EGC) which has clearer demarcation, irregular surface/vessel patterns, and specific histological features like low-grade atypia or mixed mucin expression, though subtle cancers need advanced techniques like NBI-magnifying endoscopy for accurate diagnosis.
